Outreach Intervenor Services |

Rotary
Cheshire Homes is pleased to announce that Outreach
Intervenor Services will be continuing.
Intervenors are available 7 days per
week, between the hours of 8 am to midnight, on an average
of 12 hours per week.
In addition, case management services
are available to facilitate access to community resources
and government programs and services.

Deaf-Blind
people, living in the community who have signed a service
agreement with RCH.
If you would like more information or
would like to sign up for services through RCH please
contact the Director of Outreach Intervenor Services.
Areas
of service include Toronto, Mississauga, Brampton, Woodbridge,
Thornhill, Richmond Hill, Aurora, Newmarket, Stouffville,
Markham, Pickering, Ajax, Whitby and Oshawa.

Intervenors are available for various
types of activities and are assigned according to the
needs of the consumer and the circumstance.
Intervenors can be requested for
activites of daily living such as shopping, banking, recreational,
social activities, etc.
Intervenors can also be requested for
medical, health, dental, mental health and counseling
services, employment settings, educational, legal, personal
business, and government related services.
Intervenors are requested through the
Director of Outreach Intervenor Services at RCH. Please
contact two weeks before your request date.

Outreach consumers can also use the online Intervenor request
form below. All online requests are received by the Director
of Outreach Services who will contact you to schedule your
request.
